PUBLIC NOTICE The Oregon Health Plan (OHP) is preparing to accept a limited number of new enrollees for its OHP Standard benefit package. OHP Standard provides free or low-cost health insurance to Oregon residents who: • Do not have health insurance, • Are 19 years old or older, • Are not pregnant, and • Have limited income. Because there are not enough openings to meet everyone’s needs, The Department of Human Services (DHS) is creating a new process called the OHP Standard reservation list. People interested in getting an application for OHP Standard must place themselves on the reservation list from January 28 - February 29, 2008. Beginning in March, DHS will randomly draw a predetermined number of names monthly from the list and will mail OHP Standard application forms to those whose names are drawn. From January 28 through February 29, 2008, people can place themselves, or have someone else place them, on the OHP Standard reservation list by using one of the following options: • Submit a reservation list request online at www.oregon.gov/DHS/open/request-form.shtml. • Complete and send a hard copy of the reservation list request form (OHP 3203) to DHS. The form is posted on the DHS Web site at www.oregon.gov/DHS/open/request-form.shtml and will be available at all DHS/AAA branch offices, county health departments, and most major hospitals and clinics. • Call 800-699-9075 (phone) or 503-378-7800 (TTY), and ask to be placed on the OHP Standard reservation list. The phone bank will be open Monday through Friday, 7 a.m. through 7 p.m. The call will take from 10-20 minutes. Someone may call for an individual -- they just need a full name, date of birth and mailing address. For more information about the OHP Standard reservation list, go online to: www.oregon.gov/dhs/open or visit any DHS office. Note: Only OHP Standard is closed. Other benefit packages, including those that cover pregnant women or people who are under 19 years of age, have different eligibility requirements. Individuals need to complete an OHP application if they think they may be eligible. OHP Applications are available by calling 800-359-9517 or at any DHS branch office.

CITY OF ELGIN PUBLIC NOTICE REQUEST FOR PROPOSALS Insurance Agent of Record The City of Elgin is soliciting proposals from qualified insurance agents who wish to be considered for a three-year appointment as Agent of Record. The agent(s) will obtain insurance coverages, solicited on behalf of the City, including property, auto, casualty, liability, workers’ compensation, bonds, and employee benefits. Proposals shall include information on the firm’s experience and background; information on each individual who will be responsible for managing the City’s account(s); a list of public entities that the firm has serviced; details of services the firm will provide to the City of Elgin. The city reserves the right to accept or reject any or all proposals and to waive any formalities in the selection process. The final selection of an agent(s) will be made based on the proposal(s) which in the Council’s judgement best meets the needs of the City. The City Council may accept proposals for up to 3 agents. All proposals will be reviewed by staff and forwarded to the City Council for consideration at their regular monthly meeting on February 12, 2008 at 7:00 pm. Wallowa ESD is accepting bids for the purchase and installation of Dragonwave 18GHz microwave equipment with 2.5 foot dishes between Joseph School District (N45 21.174; W117 13.360 Elevation 4,275±5) and Sheep Ridge (N45 23.955; W117 23.349; Elevation 7,021±5). This system must include redundant 18 GHz 100 Mbps link with hot standby or dual throughput at high power and all appropriate FCC licenses. Radio link must have the built-in capability/option to go to 300 Mbps. Seller must be able to provide 24/7 service, NBD parts, remote diagnostics, on site support available within 12 hours and be able to service Sheep Ridge location in adverse weather conditions such as heavy snow, wind and cold temperatures. System is to be installed on or before March 27th, 2008 with a billing date of July 1, 2008 or later. Submit sealed bids to Edward M.
