Evergreen Vista Health is currently accepting applications for a PT Activities Director for our long term care Alzheimer's/dementia facility. The Director organizes and assures the existence of an activity program that meets the activity needs of the residents and maintains a department that is in compliance with State and Federal regulations. Some duties include the development of a volunteer program, a monthly activities calendar, lead group and individual activities, provide documentation of activity assessments and provide care plan interventions.
